PCBDD provides Targeted Case Management (TCM) for developmentally disabled individuals residing in Perry County.  TCM is more commonly known as Service Coordination.  To be eligible for Service Coordination an individual must have a developmental disability defined by the state of Missouri as:

    -  A mental and/or physical disability that began before the age of twenty-two; and

    -  That is likely to continue indefinitely; and

    -  Results in substantial functional limitations in two or more of the following areas of major life activities:

                * Living and working independently
                * Communication
                * Self-Direction
                * Learning
                * Mobility
                * Self-Care

Perry County residents who have a developmental disability such as but not limited to: autism, cerebral palsy, head injury, Down syndrome, epilepsy (and other seizure disorders) and/or mental retardation may be eligible for services.
