Service Coordination

Every individual who is found eligible with the Missouri Department of Mental Health, Division of Developmental Disabilities receives Targeted Case Management (more commonly referred to as Service Coordination) services.

Service Coordinators are educated and trained in the area of developmental disabiliites and/or closely related fields.  The primary duty of a Service Coordinator is to support individuals with developmental disabilities and their families with identifying and obtaining needed services and supports.  This is done with the input of the individual, the individual's family and others who may know the individual.  Service Coordinators also evaluate and monitor the provision of authorized services to ensure the services are being provided in a manner that effectively supports the needs of the individual while also meeting Federal and State regulations.  A Service Coordinator may also act as an advocate for the individual and a source of information.
